Abstract

Persistent hypoglossal artery corresponds to a persistent fetal carotid-vertebro-basilar anastomoses in the adult that can arise from the internal carotid artery (type 1) or from the external carotid artery (type 2). It is a rare variant, especially when its origin is from the external carotid artery, with little or no clinical manifestation, but with anatomical-radiological and surgical relevance. In this case series of hypoglossal arteries, we briefly review its embryology and its different presentation variants. Its knowledge and detection are important for future endovascular procedures or surgical interventions at the skull base or in cases of carotid obstructions by complicated atherosclerotic plaques, which may cause an embolic or ischemic vascular injury in the posterior fossa. Today, its diagnosis is possible non-invasively through magnetic resonance angiography or computed tomography angiography.
